Internal Medicine/Geriatrics
Eddy SeniorCare, an established and respected PACE program (Program of All Inclusive Care for the Elderly), is seeking a permanent part-time (with the possibility of becoming full-time) Internal Medicine/Geriatric Physician to join its team. This is an excellent opportunity to join a unique practice environment which provides physicians the opportunity to work with an interdisciplinary team to support frail seniors who remain living in the community and thus avoid nursing home placement. It is a program driven by relationships -- not the number of encounters. The physician follows the participant through all care settings: the medical office, at home, in the hospital and in a nursing home should placement be needed.
Physicians interested in applying for this unique opportunity should be board certified in Internal Medicine and/or Geriatrics and have a commitment to compassionate care that is team and family oriented. Strong communications skills are a must. Experience with caring for the elderly is strongly preferred.
